2 THIS AMENDMENT, hereinafter referred to as Amendment I, is made and entered into this
3 24th day of_----'M=a=y'-----' 2016, by and between the COUNTY OF FRESNO, a Political
4 Subdivision of the State of California, hereinafter referred to as "COUNTY", and JOSEPH V.
5 MELTON, L.M.F.T. AND ASSOCIATES, a sole proprietor, whose address is 5562 W . River Bottom
6 Avenue Fresno, CA 93722, hereinafter referred to as "CONTRACTOR". Reference in this
7 Amendment to "parties" shall be understood to refer to COUNTY and CONTRACTOR, unless
8 otherwise specified.
9 WHEREAS, the parties entered into that certain Agreement, identified as COUNTY
1 0 Agreement No. 13-278, effective July 1, 2013 , whereby CONTRACTOR agreed to perform
1 1 certification hearings at various psychiatric hospitals/acute psychiatric programs in the County of
1 2 Fresno for adults, adolescents and children in order to comply with the Lanterman-Petris-Short Act as
13 set forth in the State of California Welfare and Institutions Code (W&I Code).
1 4 WHEREAS, the parties desire to amend COUNTY Agreement No. 13-278, regarding
1 5 changes as stated below and restate the Agreement in its entirety.
1 6 NOW, THEREFORE, for good and valuable consideration, the receipt and adequacy of
17 which is hereby acknowledged, the parties agree as follows:
1 8 1. That the existing COUNTY Agreement No. 13 -278, Paragraph Four (4), beginning on
19 Page Three (3), Line Twenty-Seven (27), with the word "In" and ending on Page Four ( 4), Line Three
20 (3) with the word "CONTRACTOR." be deleted and the following inserted in its place:
21 "In no event shall actual services performed under this Agreement be in excess of
22 Seventy One Thousand Two Hundred and Twenty and No/100 Dollars ($71,220.00) for each twelve
23 (12) month period of this Agreement from July 1, 2013 through June 30,2015 . In no event shall actual
24 services performed under this Agreement be in excess of One Hundred Forty Thousand and No/100
25 Dollars ($140,000) for each twelve (12) month period of this Agreement from July 1, 2015 through
26 June 30,2018. It is understood that all expenses incidental to CONTRACTOR's performance of
2 7 services under this Agreement shall be borne by CONTRACTOR."
28 2. COUNTY and CONTRACTORS agree that this Amendment I is sufficient to amend the

Agreement; and that upon execution of this Amendment I, the Agreement and Amendment I together
shall be considered the Agreement.
The Agreement, as hereby amended, is ratified and continued. All provisions, terms,
covenants, conditions and promises contained in the Agreement , and not amended herein, shall remain
in full force and effect. This Amendment I shall become effective retroactive to April 1, 2016, except
as stated above, upon execution by all parties.
