We, the above signed contractor, have given careful consideration to the change proposed and hereby agree, if
this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be
noted above, and perform all services necessary for the work above specified, and will accept as full payment
therefor the prices shown above.
If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the
specifications as to proceeding with the ordered work and filing a written protest within the time therein
specified.

Make the following changes to the plans and specifications or do work not in the plans and specifications for
this contract. Because of the change(s), ZERO WORKING DAYS will be added to the contract time.
PART I: INCREASE/DECREASE/ELIMINATE ITEMS AT UNIT PRICES:
Item quantities in the Bid Book and subsequent change orders are estimates only. This Part adjusts item
quantities to the actual quantities used.

This work did not impact the critical path for the project, so granting additional contract time was not warranted.

PART II: REMOVE AND REPLACE ASPHALT PAVEMENT AT THE WEST MATCH POINT – EXTRA
WORK
In accordance with Section 4-1.05, “Changes and Extra Work,” of the Standard Specifications, remove and
dispose of asphalt pavement (approximately 12-feet wide and 0.5-feet deep) in the eastbound lane between
Sta. 14+36 and Sta. 15+20 (west match point with the existing pavement). Grade and compact subgrade to
provide a smooth transition on either end of the pavement removal before placing the new pavement. Replace
the pavement that was removed with ½” hot mix asphalt. Perform all work according to the applicable sections
of Standard Specifications and/or Special Provisions, as directed the Engineer.
Payment for this extra work will be made using force account on a “time and materials” basis according to
Section 9-1.03, “Payment Scope,” of the Special Provisions. Payment for this work was calculated according
to Section 9-1.04, “Force Account,” of the Standard Specifications, resulting in an increase of $15,560.37.
This work did not impact the critical path for the project, so granting additional contract time was not warranted.
