09/15/2010: Amendment No. One (1) to reduce pricing per Attachment "A" attached to reflect a 10% reduction on the Polygraph
Examination Fee-Pre-Employment and to extend the contract period for one (1) additional year with an additional one (1) year automatic renewal.
09/04/2012: Increase maximum contract amount to One Hundred Thousand dollars ($100,000.00).
SPECIAL CONDITIONS FOR USE:
*AUTOMATIC RENEWAL: In the absence of notification from vendor to Purchasing, or instructions from County department to Purchasing, this Agreement
will automatically renew for one year on November 1, 2012 and November 1, 2013.

Contract Number P-09-474-K covering Polygraph Testing Services is amended as follows:
Amend the paragraph entitled MAXIMUM to "In no event shall the total services
performed and/or fees paid under this Agreement be in excess of one hundred thousand
dollars ($100,000.00)".
This amendment will be effective immediately. Prices will be firm for the contract period.
Amendment provisions shall remain in full force and effect with any contract renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter
to my office, retaining a copy for your files.
