Hamilton Ave. Fresno, CA 93702 ATTN: CAROLYNFLORES Attached is the equipment list and current pricing for the annual Preventive Maintenance Agreement (#P-11-532-H), effective September 1, 2014 through August 31,2015. We are pleased to report that our prices have not changed from last year, with the exception of microscopes and autoclaves. As always, repairs and parts are not included in the Agreement, but can be requested on an as needed basis through separate purchase orders. The items do not need to be listed in the Agreement. Repair costs remain at a rate of $95.00 per hour (with a minimum of one hour), plus a travel charge of$95.00 per service call. Overtime charges begin after 5 p.m. with a rate of $142.50 per hour (1 1/2 times regular rate) and travel charge of $142.50 per service call. We have maintained an ongoing relationship of quality, integrity and dependability with many of Fresno County's staff.
