Contract Highlights

This is a three-year negotiated agreement. Shamrock will continue to provide blank and pre-printed labels, stock and custom at
a 25% discount. This negotiated agreement adds wristband products from LaserBand and Precision Dynamics.
Update Highlights
This contract has been renegotiated effective October 1, 2014 for three years through September 30, 2017. New price list is
attached and the CDS has been revised.
The price ftle has been updated to include Shamrock Label items. The current pricing will remain the same; Numerous items
have been added to the price ftle.
Precision Dynamics and Laserband items will be removed from the current contract. Precision Dynamics products are carried
under a separate agreement. Shamrock no longer sells Laserband items under this agreement. Shamrock has added another line
of wristbands in the place of Precision Dynamics or Laserband items.

the contract vendor or its authorized distributor(s) to Amerinet, Inc. to provide funding for continued program development
and administration. Amerinet, through the organization with which you are afftliated, will disclose in writing to your facility, at
least annually, the amount received from the above and/or its authorized distributor with respect to purchases made by or on
behalf of your facility.

RETURN POLICY
NO RETURNS WILL BE ACCEPTED WITHOUT PRIOR AUTHORIZATION.
SHAMROCK'S NUMBERED RETURN AUTHORIZATION FORM MUST
ACCOMPANY ALL MERCHANDISE RETURNED.
DEFECTIVE MERCHANDISE: SHOULD ANY SHAMROCK PRODUCT PROVE
TO BE DEFECTIVE, A CLAIM FOR RETURN MUST BE PLACED WITHIN 60
DAYS OF SHIPMENT. PRODUCTS SHOWN TO BE DEFECTIVE AND
RETURNED WITHIN THIS PERIOD WILL BE REPLACED.
STOCK ITEMS: NO RETURN OF STOCK ITEMS WILL BE ACCEPTED AFTER
SIX MONTHS. A 15% RESTOCKING CHARGE IS IMPOSED ON RETURN OF
ALL STOCK ITEMS.
CUSTOM ITEMS: CUSTOM ORDERS MAY NOT BE RETURNED.
