Contract Number P-14-034-G, Attachment "A" covering Janitorial Services at Coalinga-Huron
is hereby amended to include the all labor, material, equipment, taxes, insurance, bonds, etc. to
provide two additional days of janitorial services as specified for the following location from RFQ
485-5218:
36678 5. Lassen Ave #1
Huron, CA 93234
The County of Fresno hereby accepts the additional cost of $4,800.00, which will increase the
annual cost at the Huron location to a total of $12,600.00 annually to be effective immediately
in accordance with the terms and conditions of our Contract Number P-14-034-G and Bid
Number 485-5218. All prior orders will be priced in accordance with current contract
pricing.
In no event shall services performed and/or fees paid under this Agreement be in excess of one
hundred thousand dollars ($1 00,000.00).
This amendment will be effective from immediately through March 31, 2015. Prices will be firm
for the contract period. Amendment provisions shall remain in full force and effect with any
contract renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter
to my office, retaining a copy for your files.
