Contract Number P-12-513-M covering Lease Ground Level Office and Modular Complex is
amended to include the following:
The County of Fresno desires to extend this contract for an additional one (1) year
effective October 1, 2014 through September 30, 2015.
The County of Fresno hereby accepts your price increase notice of fifteen dollars
($15.00) per month for Unit# G40015, Serial# 806229-6 and fifty-five dollars ($55.00)
per month for Unit #3251-3252, Serial #17112-17113 to be effective October 1, 2014 in
accordance with the terms and conditions of our Contract Number P-12-513-M.
All orders placed prior to October 1, 2014 will be priced in accordance with current
contract pricing.
In no event shall services performed and/or fees paid under this Agreement be in excess of
seventy-five thousand dollars ($75,000).
Prices will be firm for the contract period. Amendment provisions shall remain in full force and
effect with any contract renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter
to my office, retaining a copy for your files.
