Contract Number P-16-097-S covering Identification Wristbands, Closures and Equipment is amended
as follows:
The contract with your company expires March 31, 2021. The County of Fresno desires to extend this
contract for an additional three (3) months period effective April 1, 2019 through June 30, 2021. Prices
will be firm for the contract period. Amendment provisions shall remain in full force and effect with any
contract renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter to my
office, retaining a copy for your files.
