Accessories is amended to include the items and pricing listed on Attachment A
This amendment will be effective from May 1, 2015 through September 30, 2016. Prices will be
firm for the contract period. Amendment provisions shall remain in full force and effect with any
contract renewals.
In no event shall services performed and/or fees paid under this Agreement be in excess of
seven hundred twenty-one thousand, six hundred fifty dollars ($721 ,650.00).
Please acknowledge your acceptance by returning all pages of the signed original of this letter
to my office, retaining a copy for your files.
