Contract Number P-13-008-M providing Online Training Services for the Prevention of
Workplace Harassment is amended as follows:
MAXIMUM: In no event shall services performed and/or fees paid under this agreement
be in excess of fifteen thousand seven hundred and fifty dollars ($15, 750.00).
This amendment will be effective immediately through December 31, 2015. Prices will be firm
for the contract period. Amendment provisions shall remain in full force and effect with any
contract renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter
to my office, retaining a copy for your files.
