We, the above signed contractor, have given careful consideration to the change proposed and hereby agree, if
this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be
noted above, and perform all services necessary for the work above specified, and will accept as full payment
therefor the prices shown above.
If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the
specifications as to proceeding with the ordered work and filing a written protest within the time therein
specified.
TO: EMMETTS EXCAVATION, INC:
Make the following changes to the plans and specifications or do the following work not included in the plans
and specifications for this contract. Because of the change(s), ZERO WORKING DAYS will be added to the
contract time.
MODIFY SPECIAL PROVISIONS SECTION 14-12.04
Delete Special Provision Section 14-12.04 in its entirety and replace it with:
14-12.04 RELATIONS WITH SAN JOAQUIN VALLEY AIR POLLUTION CONTROL DISTRICT (SJVAPCD)
You are responsible for compliance with all applicable SJVAPCD regulations and requirements. This section is
provided for your information, and nothing herein or elsewhere within these special provisions shall be
construed as limiting your responsibility for complying with all applicable rules and regulations.
Section 6.4 of District Rule 8021, District Notification of Earthmoving Activities on Smaller Construction Sites,
an owner/operator of a construction project of at least 1.0 acre in size shall provide written notification to the
District at least 48 hours prior to his/her intent to commence any earthmoving activities. There are no fees for
filing a construction notification.
This change does not impact the critical path for the project, so granting additional contract time is not
warranted.
