Management __ RE/Arch. __ Design We, the above signed contractor, have given careful consideration to the change propo sed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefor the prices shown above. If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the specifications as to proceeding with the ordered work and filing a written protest within the time therein specified. TO: AMERICAN PAVING CO.: You are hereby directed to make the herein described changes from the plans and specifications or do the following described work not included in the plans and specifications on this contract. By reason of the proposed change, ZERO WORKING DAYS will be granted to the allotted contract time. PART I: DEDUCTION FOR EXTRA WELDING INSPECTION SERVICES Deduct from the contract the cost for additional welding inspection services on June 30, 2020, due to the replacement of girder shear studs, according to Section 6-2.03, Department Acceptance of the Standard Specifications. The actual cost of the inspection service incurred by the Department was $1,121 of which will be deducted from the monies due to the contract. Total Cost Part I: $1,121 DECREASE PART II: DEDUCTION FOR AN EXTENDED FULL ROAD CLOSURE ON JUNE 2, 2020 Deduct from the contract, for public inconvenience, damages due to a road closure starting the morning of June 2 and extending through the late afternoon of June 3, 2020. The contract allows for road closures up to eight hours during a working day, with no road closures allowed beyond this duration. This part finalizes the deduction for the overnight road closure in the amount of $4,623.00.
