Contract Number P-14-365-0 covering Medical Waste Transport and Disposal services is amended as
follows:
Amend and include services to Fresno County Department of Behavioral Health locations. The
department contact is Ike Grewal at 600-2561 . See the attached modifications to contract Quotation
Schedule. This amendment will be effective December 1, 2016 through the expiration date of the
agreement. Amendment provisions shall remain in full force and effect with any contract renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter to my
office, retaining a copy for your files.
