Re: One-Year Extension of Agreement#17-196
Dear Mr. Cotham
The above Agreement for the provision of Internship Experience for Students in Adminishative
Disciples between the County of Fresno and California State University, Fresno (CSUF), which
was approved by the Board of Supervisors on May 16,2017 , allowed for the option of four one-
year extensions upon written approval of both parties no later than thirty days prior to the first
day of the next twelve-month extension period June 1, 2020 andauthorized the Department of
Public Health Director or designee to execute such written approval on behalf of the County.
I have reviewed the perfomance of CSUF in providing the contracted services and find it
continues to be satisfactory. Therefore, I would like to extend the agreement form June 1,2020
through May 31,2021. If you, as the signing authority for CSUF, would also like the agreement
to be extended as stated, please sign and date on the line below and retum this letter to:
