AMENDMENT I TO AGREEMENT
THIS AMENDMENT, hereinafter referred to as Amendment I, is made and entered into this
___ day of ______ , 2016, by and between the COUNTY OF FRESNO, a Political
Subdivision of the State of California, hereinafter referred to as "COUNTY", and each contractor listed
in Exhibit A-1, attached hereto and by this reference incorporated herein, collectively hereinafter
referred to as "CONTRACTORS" and such additional contractors as may, from time to time during
the term of this Agreement, be added by COUNTY's Department of Behavioral Health Director, or
designee. Reference in this Amendment to "parties" shall be understood to refer to COUNTY and each
individual CONTRACTOR, unless otherwise specified.
WHEREAS, the parties entered into that certain Agreement, identified as COUNTY
Agreement No. 13-241, effective May 7, 2013, whereby CONTRACTORS agreed to provide licensed
psychiatrists.
WHEREAS, the parties desire to amend COUNTY Agreement No. 13-241, regarding
changes as stated below and restate the Agreement in its entirety.
NOW, THEREFORE, for good and valuable consideration, the receipt and adequacy of
which is hereby acknowledged, the parties agree as follows:
1. That the existing COUNTY Agreement No. 13-241, Section Five (5), beginning on Page
18 Five (5), Line Seventeen (17), with the word "For" and ending on Page Five (5), Line Twenty-One
19 (21) with the word "Dollars ($150,000.00)." be deleted and the following inserted in its place:
20 "For the period of July 1, 2013, through June 30, 2018, the maximum compensation
21 under the terms of this Agreement shall not exceed One Million Eight Hundred Thousand and No/100
22 Dollars ($1,800,000.00). For the period of July 1, 2013 through June 30, 2014, the maximum
23 compensation under the terms of this Agreement shall not exceed One Hundred Fifty Thousand and
24 No/100 Dollars ($150,000.00). For the period of July 1, 2014 through June 30, 2015, the maximum
25 compensation under the terms of this Agreement shall not exceed One Hundred Fifty Thousand and
26 No/100 Dollars ($150,000.00). For the period of July 1, 2015 through June 30, 2016, and each twelve
27 (12) month period beginning July 1, 2016 through June 30, 2018, the maximum compensation under
28 the terms ofthis Agreement shall not exceed Five Hundred Thousand and No/100 Dollars
