We, the above signed contractor, have given careful consideration to the change proposed and hereby agree, if
this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be
noted above, and perform all services necessary for the work above specified, and will accept as full payment
therefor the prices shown above.
If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the
specifications as to proceeding with the ordered work and filing a written protest within the time therein
specified.

Make the following changes to the plans and specifications or do the following work not included in
the plans and specifications for this contract. Because of the change(s), ZERO WORKING DAYS will
be added to the contract time.

Bid Item No. 14 Temporary Fence (Type ESA) decreased by 80.00 percent because the Environmentally
Sensitive Area did not require the use of 160 linear feet. You did not make a request for adjustment to the unit
price and the actual quantities were paid at the unit price at bid opening, resulting in a decrease of $1,600.
These changes do not affect the controlling activity. No time adjustment is warranted.

PART II: EXTRA WORK
In accordance with Section 4-Standard Specifications construct
a 6-foot by 6-foot junction box and lid instead of the planned 5-foot by 5-foot junction box. The
construction of the larger junction box is necessary to accommodate the larger 54-inch Class III
RGRCP in the Outside Drain Canal. Construct the box using the FMFCD detail for the Type N Outfall
Structure for a 72-inch square cast-in-place box. The top of the box will be constructed using the detail
shown on Plan Sheet C8. Payment for this change will be made as extra work at an agreed price of
$932.90 in addition to the Junction Box and Lid
according to Section 9-1.03, This total was verified using
a force account analysis using the methods in Section 9-Standard
Specifications. This price includes compensation for additional labor, materials, equipment, and
markups for this change.
These changes do not affect the controlling activity. No time adjustment is warranted.

TOTAL TIME EXTENSION PART IV: ZERO WORKING DAYS
PART III: CONSTRUCT A SQUARE 9 ARV VAULT AND TRAFFIC-RATED LID EXTRA
WORK
In accordance with Section 4-Standard Specifications
construct a 9-foot by 9-foot air release vault and traffic-rated lid instead of the planned 6-foot by 6-
inch square pre-cast vault shown on Plan Sheet C9. Relocate the proposed air vent to the back of
right-of-way and in line with the telephone pole shown on Plan Sheet C9. Payment for the additional
cost of this change will be made as extra work at an agreed price of $35,456.58 in addition to the
payment , in accordance with Section 9-1.03,
, This total was verified using a force account analysis
using the methods in Section 9-Standard Specifications. This agreed
lump sum prince includes compensation for all additional costs, including, materials, equipment,
labor, incidentals, overhead, delays if any, and markups.
This change does not affect the controlling activity. No time adjustment is warranted.

PART IV: CONSTRUCT A STABILIZATION LAYER UNDER THE BOX CULVERT, INLET, AND
OUTLET INVERTS EXTRA WORK
In order to place formwork and reinforcing bars for the box culvert, inlet, and outlet inverts, construct a
stabilization layer to minimize the amount of subgrade instability caused by water being transmitted to
the subgrade through the wick drains. According to Section 4-1.05,
Standard Specifications, stabilize the subgrade for the box culvert. Place a layer of geotextile
equivalent to the material previously used onsite, and 6-inches of crushed gravel below the subgrade
of the box culvert and west of the box culvert at the cut-off wall to stabilize the foundation. This work
will be paid as extra work at a time and materials basis. Payment for this change calculated
according to Section 9-1.04 This price
includes compensation for all labor, materials, equipment, overhead, and overtime markups for this
change.
This change affect ed the controlling activity but not enough to impact the project schedule. No time
adjustment was requested.

PART V: INCREASE THE HEIGHT OF THE NUISANCE BERMS EXTRA WORK
According to Section 4-Standard Specifications, increase the
height of the nuisance berms by 3 feet. The additional soil will be necessary to build up the nuisance
berms to withstand the additional 3-feet of water upstream. This work will be paid as extra work on a
time and materials basis. Payment for this change calculated according to Section 9-1.04
of the Standard Specifications is $1,931.96. This price includes compensation for all labor,
materials, equipment, overhead, and markups for this change.
These changes do not affect the controlling activity. No time adjustment is warranted.

PART VI: PAYMENT FOR MATERIALS THAT COULD NOT BE RETURNED FOR BID ITEM NO.
EXTRA WORK
This bid item was reduced because the return walls were omitted during construction to allow time for
further discussion around the Dos Palos Joint Powers Authority water main pipeline. You incurred a
cost of $4,845.78 including appropriate markups for the materials on hand that could not be reused or
returned to the supplier. The cost of this material will be paid as extra work according to Section 9-
ial Provisions.

PART VII: MOVE UNSUITABLE NATIVE SOIL FOR CCID EXTRA WORK
In accordance with Section 4-Standard Specifications regrade the
canal embankments for Central California Irrigation District with the unsuitable native soil excavated below the
original grade. Grade the canal embankments to match the existing conditions. This work will be paid as extra
work at a time and material basis. Payment for this change calculated according to Section 9-
Standard Specifications is $10,050.08. This price includes compensation for all labor,
materials, equipment, overhead, and markups for this change.
These changes do not affect the controlling activity. No time adjustment is warranted.
