The Agreement referenced above allows for the option of two additional consecutive
twelve-month period upon written approval of both parties and authorizes the Director
of Social Services to execute such written approval on behalf of the Department.
The Department is seeking to extend the agreement from July 1,2020 through June
30,2021.If you concur,please sign and date on the following page and return this
letter by May 28,2020 to:
