Management __ RE/Arch. __ Design We, the above signed contractor, have given careful consideration to the change proposed and hereby agree, if this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be noted above, and perform all services necessary for the work above specified, and will accept as full payment therefor the prices shown above. If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the specifications as to proceeding with the ordered work and filing a written protest within the time therein specified. TO: DAWSON-MAULDIN, LLC: Make the following change to the plans and specifications or do the following work not included in the plans and specifications. Because of the change(s), ONE WORKING DAY will be added to the allotted contract time. The revised completion date is November 26, 2020. PART I: REPLACE THE VALVE AT THE WATER TANK CONNECTION FOR THE TEMPORA SUPPLY LINE EXTRA WORK In order to accommodate the 6-inch backflow preventer and the 6-inch HDPE bypass line, replace the valve on the water tank connection with a 6-inch Mueller gate valve with a concrete support pad and permanent pipe support. The Section 99-1.02B, of the Special Provisions, does not address this change. Payment for the additional cost you incurred will be made as extra work at an agreed lump sum price of $2,490.44. This agreed lump sum price includes all compensation for the additional costs incurred, including additional labor, materials, equipment, overhead, incidentals, delays if any, and markups. This work impacts the critical path for the project, so one additional contract day is warranted.
