4 . The parties mutually agree to this amendment as follows . All actions noted below are by th is reference made a part of the Agreement and
incorporated herein :
This contract is being amended to add Dreamcatchers as the case service contractor (CSC) to provide employment services, and to allow shifting of
funding . The following exhibits have been amended; Exhibit A. l -the Scope of Work and Exhibit B. l -Program Budget Match Summary, DOR Program
Budget and the Contractor Service Budget. The total amount of this agreement has decreased from $1,784,883 .00 to $1,340,597 .00; a decrease of
$444,286 .00 . This reduction is due to the Contractor's Service Budget decreasing from $594,961 to$ 150,675 in FY 2019 /20; the $444,286 .00 in Service
Budget reduction has been shifted to the case service contract. This has increased the DOR Budget by the amount of the CSC Service Budget for a total
of $758,821 .00 .
The effective date of the amendment is January 1, 2020.
All other terms and conditions shall remain the same.
