AMENDMENT II TO AGREEMENT
THIS AMENDMENT TO AGREEMENT (hereinafter “Amendment II”) is made and entered into
this ______ day of ______, 2020, by and between COUNTY OF FRESNO, a Political Subdivision of
the State of California, Fresno, California (hereinafter “COUNTY”), and K&L Clinical Forensic Practice,
a California corporation, whose address is 3447 W. Shaw Avenue, Fresno, California 93711
(hereinafter “CONTRACTOR”).
WITNESSETH:
WHEREAS, COUNTY and CONTRACTOR entered into Agreement Number P-19-231-Q,
dated May 1, 2019, (hereinafter “Agreement”), as amended by Amendment Number One (1) to
Agreement Number P-19-231-1-Q, dated December, 9, 2019, hereinafter collectively referred to as
Agreement Number P-19-231-1-Q, pursuant to which CONTRACTOR agreed to provide psychiatric
evaluation and court testimony services for Lanterman-Petris-Short (LPS) Act Conservatees as
defined by the Welfare and Institutions Code Section 5350, and reports for individuals deemed to be
mentally incompetent to stand trial (IST) as defined by Penal Code Section 1368, as well as additional
services required by COUNTY; and
WHEREAS, COUNTY and CONTRACTOR now desire to amend the Agreement in order to
extend the term of the Agreement.
NOW, THEREFORE, for good and valuable consideration, the receipt and adequacy of which
is hereby acknowledged, COUNTY and CONTRACTOR agree as follows:
1. That in the existing Agreement Number P-19-231-Q, as set forth in the original
Agreement Section Three (3) “TERM”, located on Page Three (3), shall be revised by adding the
following at Line Five (5) after the word “term”:
"This Agreement shall be extended for an additional three (3) month period beginning
July 1, 2020 and shall terminate on September 30, 2020.”
2. COUNTY and CONTRACTOR agree that this Amendment II is sufficient to amend the
Agreement and, that upon execution of this Amendment II, the Agreement, Amendment I, and
Amendment II together shall be considered the Agreement.
The Agreement, as hereby amended, is ratified and continued. All provisions, terms,

covenants, conditions and promises contained in the Agreement and not amended herein shall remain
in full force and effect.
