Dear Ms. Hasan:
The Agreement referenced above, which was approved by the Board of Supervisors on
July 11, 2017, allowed for the option of two one-year extensions upon written approval
of both parties and authorized the Director of Social Services or his designee to execute
such written approval on behalf of the County.
The Department is seeking to extend the agreement from July 1, 2020 through June 30,
2021. Please indicate yes or no on the following page, sign and date, scan, and return
