HomeMy WebLinkAbout2415C CCO 05.pdf COUNTY OF FRESNO CONSTRUCTION MANAGEMENT CONTRACT CHANGE ORDER CSA 1 TAMARACK WATER INFRASTRUCTURE CHANGE ORDER NO. 5 REPLACEMENT OCTOBER 10, 2025 CONTRACT NO. 24-15-C PAGE 1 OF 2 TO: DG CONCTRUCTION, INC.: Make the following changes to the plans and specifications or do work not in the plans and specifications for this contract. Because of the change(s), SIX(6) -WORKING DAYS will be added to the contract time. PART I: ADJUST COMPENSATION FOR THE USE OF PEA GRAVEL INSTEAD OF SAND FOR PIPE BEDDING This change was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications, use 3/8"-1/2" pea gravel as the placement for bedding in the bottom of trench instead of sand bedding to place 6-inch HDPE pipe on Flintridge Road and Cordwood Road. Payment for the additional cost will be paid at an agreed unit price of$35.00 per ton for the difference of cost of pea-gravel ($52.00 per ton pea-gravel) to sand ($17.00 per ton) being placed, according to Section 9-1.03, "Payment Scope," of the Special Provisions. A total of 69.05 tons of 3/8"-1/2" pea gravel was delivered to the job site; the additional compensation resulted in (69.05 tons x $35.00 per ton = $2,416.75) $2,416.75. This amount covers trucking, labor, and placing material. Total compensation for this work resulted in a $2,416.75 increase, this amount will be paid through Bid Item No. 1, Supplemental Work with no net increase to the contract amount. Four(4) working days will be added to the contract time for the above referenced extra work, in accordance with Section 8-1.07B, "Time Adjustments," of the Project Specifications TOTAL COST PART I: $0.00 (PAID IN BID ITEM NO. 1) TOTAL TIME EXTENSION PART I: FOUR (4) -WORKING DAYS PART II: REPAIR A PREEXISITNG LEAKY WATERMAIN AT 15+00 (CORDWOOD ROAD)—EXTRA WORK This extra work was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications, repair a leaky watermain at Sta. 15+00 (Cordwood Road). Perform all work according to the applicable sections of the Special Provisions and/or Standard Specifications, as directed by the Resident Engineer. Payment for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope,"of the Special Provisions. Total compensation for this work resulted in a $3,601.64 increase, this amount will be paid through Bid Item No. 1, Supplemental Work with no net increase to the contract amount. Two (2) working days will be added to the contract time for the above referenced extra work, in accordance with Section 8-1.07B, "Time Adjustments," of the Project Specifications. Contractor We,the above signed contractor, have considered the change proposed and hereby agree,if this proposal is _Auditor Controller approved,that we will provide all equipment,furnish all materials,except as may otherwise be noted above, Business office and perform all services necessary for the work above specified, and will accept as full payment therefore the Board of Supervisors _Const.Management Prices Shown above. RE/Arch. _Design If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the specifications as to proceeding with the ordered work and filing a written protest within the time therein specified.
