HomeMy WebLinkAboutSTATE DPH SNAP-Ed CalFresh Healthy Living Program_A-23-428-2.pdf A-23-428-2 September 16, 2025 TO: CalFresh Healthy Living Local Health Departments SUBJECT: Program Letter 25-06 California Department of Public Health CalFresh Healthy Living Federal Fiscal Year 2025 and 2026 Guidance This document serves as an addendum to the Federal Fiscal Year (FFY) 2024 - 2026 California Department of Public Health (CDPH) CalFresh Healthy Living Funding Application Package and supersedes the requirements and deliverables therein. The purpose of Program Letter (PL) 25-06 is to provide FFY 2025 and 2026 requirements and guidance for Local Health Departments (LHDs) to complete and close out local CalFresh Healthy Living programs. The FFY 2026 CDPH CalFresh Healthy Living period will run from October 1, 2025, to April 30, 2026. The focus of the work is program wind-down. Beginning October 1, 2025, LHDs should begin to phase out direct services, including, but not limited to, direct and indirect education to participants. LHD staff are encouraged to work with partners and within their own agencies to identify the most impactful activities and pursue strategies, when possible, to maintain and/or sustain nutrition and physical activity work. The United States Department of Agriculture will not issue Supplemental Nutrition Assistance Program-Education (SNAP-Ed) Plan Guidance for FFY 2026, therefore LHDs must adhere to the allowable activities outlined in the FFY 2025 SNAP-Ed Plan Guidance. LHDs must complete all required FFY 2025 and FFY 2026 reporting requirements. The following tables contain: 1) required FFY 2025 year-end activities; 2) required FFY 2026 program reporting transition activities; 3) required FFY 2026 administrative and fiscal transition activities, and 4) suggested FFY 2026 transition and sustainability activities. Invoice should be clearly marked "Final Invoice," indicating that all payment obligations of the State under this grant have ceased and that no further payments are due or outstanding Table 4: Suggested FFY 2026 Programmatic Transition Activities CDPH CalFresh Healthy Living acknowledges the differences and uniqueness of each local CalFresh Healthy Living program. The activities below are based on recommendations and discussions with LHDs about the relevant and/or necessary activities within jurisdictions. LHD staff are advised to review and consider the list below, then decide the best way to wind down your local program. PEARS may not capture all transition activities so there may be a need for supplemental data reporting processes. 2 If an LHD chooses to end the grant agreement prior to April 30, 2026, via writing to NPABfiscal(a_cdph.ca.poi, a formal closeout letter will be issued with revised due dates for required deliverables. Confidential - Low September 16, 2025 TO: CalFresh Healthy Living Local Health Departments SUBJECT: Program Letter 25-06 California Department of Public Health CalFresh Healthy Living Federal Fiscal Year 2025 and 2026 Guidance Transition and Sustainabilit Activities for Consideration Identify key CalFresh Healthy Living program elements to transition/share, such as: • Materials (curricula, printed resources, fact sheets, work plans) • Coalition and/or partnership documents • Physical resources (equipment, etc. Identify and communicate with organizations that may continue nutrition and physical activity work and be open to sustaining/adopting program elements.3 Organizations that might support sustain ing/transitioning CalFresh Healthy Living work may include, but not be limited to: • An LHD program with alternative funding source(s) • Community-based organizations • Schools • Current and/or new partners (including other Local Implementing Agencies). • Subcontractors Distribute CalFresh Healthy Living materials, toolkits, and other resources available through the Web StoreFront to partners and provide social media/website/newsletter/digital content to other applicable programs. • Download and save/share relevant web-based materials for local use Resources of local value should be housed somewhere locally due to the uncertainty of availability after the program ends Develop and implement a sustainability plan that includes activities such as: • Implementing a Train-the-Trainer program for direct education and policy, system, and environmental change (PSE) strategies with local partners and LHD staff not previously trained on CalFresh Healthy Living materials and approaches • Completing final Site Level Assessment Questionnaires/Organizational Assessment Questionnaires in Survey123 to demonstrate accomplishments and aid in sustainability planning • Conducting exit interviews with community champions to: o Help them find a new outlet to continue their efforts o Describe the impact of CalFresh Healthy Living interventions in the jurisdiction o Prioritize elements of the CalFresh Healthy Living program to continue o Identify others in the jurisdiction that might continue the CalFresh Healthy Living work 3 It is not required or expected that any partners or LHDs will replicate the existing program at any scale. Confidential - Low September 16, 2025 TO: CalFresh Healthy Living Local Health Departments SUBJECT: Program Letter 25-06 California Department of Public Health CalFresh Healthy Living Federal Fiscal Year 2025 and 2026 Guidance • Engaging partners in efforts to continue the legacy of the CalFresh Healthy Living program (e.g., incorporate CFHL goals and objectives into other local coalition work) • Promoting program accomplishments (e.g., via media, online and funder outreach) • Identifying and applying for other similarly focused funding sources • Presenting CalFresh Healthy Living program accomplishments to decision makers to garner support for future program funding Document the history, activities, and successes of the CalFresh Healthy Living program in your jurisdiction and capture the anticipated impact of the loss of funding. Relevant descriptions, documents, and activities may include: • Program focus (e.g., community assessment and need) • Future recommendations (e.g., policy and programmatic focuses) • Partners • Site list • Budget • Integrated Work Plan • Direct education curricula used • PSE strategies implemented • Training resources • An archive of past reports and success stories • Local level program data (e.g., PEARS, Site Level Assessment Questionnaires, Impact Outcome Evaluation, Adult Education Evaluation) • Sharing your perspectives and experiences with the CDPH and Nutrition Policy Institute (NPI) about the anticipated impact of the loss of funding on your LHD and community partners via surveys, interviews, and/or focus groupS4 Consider accessing 1:1 TA from the Partnership and Coalition Statewide Coordinating Center and Policy Statewide Coordinating Center to support your activities There is no requirement for LHDs to return supplies, including computer equipment, purchased with SNAP-Ed funds to CDPH. Starting Oct. 1, 2025, please email NPABFiscal(a_cdph.ca.gov for administrative and fiscal questions, and email LASS.TA(a-_)cdph.ca.gov for programmatic support. 4 There is intention for CDPH and its evaluation subcontractor, NPI, to facilitate formal collection of perspectives from LHDs and community partners to understand the impact of the loss of SNAP-Ed funding in communities.
