RE: One-Year Extension of Purchasing Agreement No. 23-486
Dear Mr. Josefovic,
Purchasing Agreement No. 23-486, with Streamline Verify LLC for the provision of Social
Security Administration (SSA) Death Master File (DMF) Database was approved by our
Purchasing Manager on September 13, 2023, and allows for the option of four one-year
extensions upon written approval of both parties and authorized our Director to execute such
action on behalf of the County.
Department staff have reviewed the performance of Streamline Verify LLC in providing the
contracted services and find it continues to be satisfactory. On June 9, 2025, our department
expressed our intent to renew the agreement from July 1, 2025, through June 30, 2026, via
a telephone call between our Plan Administration Division Manager (Joseph Rangel) and
Dov Schecter from your organization. To memorialize the renewal, please sign and date
below and e-mail a copy of the signed letter to Estevan Gonzalez, Staff Analyst at:
egonzalez(a_fresnocountyca.gov at your earliest convenience.
