1 AMENDMENT I TO AGREEMENT
2 THIS AMENDMENT, hereinafter referred to as "Amendment I'', is made and entered into this
3 __ day of__;_;Mc:.::a:.~..y __ , 2016, by and between the COUNTY OF FRESNO, a Political Subdivision
4 ofthe State of California, hereinafter refeiTed to as "COUNTY", and CENTRO LA FAMILIA,
5 whose address is 302 Fresno Street, Fresno, California, hereinafter referred to as "CONTRACTOR.".
6 WHEREAS, the parties entered into that certain Agreement, identified as COUNTY Agreement
7 No. 11-338, effective July I, 2011, hereinafter referred to as "Agreement", whereby CONTRACTOR
8 agreed to provide a Mental Health Services Act (MHSA) funded Consumer/Family Advocacy
9 Consultant for the provision of advocacy, support, and other services to consumers and their families
10 receiving or in need of County mental health services; and to act as liaison between consumers, family
11 members, and other support systems and County professional staff and other groups or organizations
12 in the community; and
13 WHEREAS, the parties now desire to amend the Agreement regarding changes as stated below
14 and restate the Agreement in its entirety.
15 NOW, THEREFORE, for good and valuable consideration, the receipt and adequacy of which
16 is hereby acknowledged, the parties agree as follows:
17 1. This Agreement shall be extended for an additional six (6) month period beginning July
18 1, 2016 through and including December 31, 2016 upon the same terms and conditions herein set
19 forth.
20 2. That the following be inserted in Section Four (4) "COMPENSATION", Page Four (4),
21 Line One (1) of the existing COUNTY Agreement No. 11-338:
22 "The maximum amount for the extended period of July 1, 2016 through
23 December 31, 2016 shall not exceed Fifty-Six Thousand Seven Hundred Eighty-Five and No/1 00
24 Dollars ($56,785.00), as set forth in the budget, attached hereto as Extended Exhibit C and
25 incorporated herein by reference."
26 COUNTY and CONTRACTOR agree that this Amendment I is sufficient to amend the
27 Agreement; and that upon execution of this Amendment 1, the Agreement and Amendment I together
28 shall be considered the Agreement.
