Contract Number P-24-253 covering system of care leadership technical assistance and coaching is
amended as follows:
Attachment"A" in its entirety, dated June 7, 2024, is hereby replaced with Revised Attachment"A" dated
September 23, 2025.
Electronic Signatures. The parties agree that this Agreement may be executed by electronic signature as
provided in this section.
A. An "electronic signature" means any symbol or process intended by an individual signing this
Agreement to represent their signature, including but not limited to (1) a digital signature; (2) a faxed
version of an original handwritten signature; or(3)an electronically scanned and transmitted (for
example by PDF document)of a handwritten signature.
B. Each electronic signature affixed or attached to this Agreement(1) is deemed equivalent to a valid
original handwritten signature of the person signing this Agreement for all purposes, including but not
limited to evidentiary proof in any administrative or judicial proceeding, and (2) has the same force
and effect as the valid original handwritten signature of that person.
C. The provisions of this section satisfy the requirements of Civil Code section 1633.5, subdivision (b), in
the Uniform Electronic Transaction Act(Civil Code, Division 3, Part 2, Title 2.5, beginning with section
1633.1).
D. Each party using a digital signature represents that it has undertaken and satisfied the requirements
of Government Code section 16.5, subdivision (a), paragraphs (1)through (5), and agrees that each
other party may rely upon that representation.
This Agreement is not conditioned upon the parties conducting the transactions under it by electronic
means and either party may sign this Agreement with an original handwritten signature.

System of Care Leadership Technical Assistance and Coaching
Deliverable Description Hours
1. Leadership Engagement and 0 Up to 3 half day, facilitated Learning Conversations for all ILT, 70
Learning Conversations EAC and other senior leaders (Max 120 attendees)
• Small Group/Breakouts for System Leaders to explore
commitments and visioning for"Large S" System of Care
• Development of materials and handouts, in partnership with
Fresno County leads
• Interview with other key stakeholders.
Purposes:
• Establish shared understanding of System of Care mindset,
purpose and implementation drivers
• Understand the necessary factors and conditions for change for
implementation success (Whole Child/Healing Focused)
• Understand need for Integrated Core Practice Model as shared
practice guide for system partners
• Recognize that MOU/System of Care is not"an initiative", but a
blue print for all other initiatives
2. Ongoing Coaching and Technical . Agenda consult/formation for leadership meetings 93
Assistance to System of Care ILT 0 Observation of current meeting processes
and EAC Teams . Assessment of key processes and functions of the existing system
leadership—Is the MOU useful/working?
• Review of system's administrative support processes
• Exploration and potential use of ILT Self-Assessment Tools
Initial brainstorming of System of Care Dashboard
• Leadership Training in Integrated Core Practice Model Guide
• Supporting development, review and implementation of State-
required plans and initiatives
Offering coaching and capacity-building to strengthen System of
Care infrastructure, leadership processes, and interagency
collaboration
• Providing consultation and technical assistance for initiatives
addressing the needs of complex youth

Total Hours 246 @$225/hour. Consultation cost estimate of$55,350.00
Travel allowance of$3,200, including hotel and milage. Total contract cost not to exceed $58,550.00
Note: At DSS discretion and direction, hours may be reallocated to other deliverables as needed.
