A.Agreement extended for twelve(12)months with the effective date of this amendment being July 1,2024,or upon DSH approval.
B.This Agreement does not increase the original dollar amount of$5,843,700.00. The total maximum amount of this Agreement shall remain at
$5,843,700.00.
All other terms and conditions shall remain the same. ATTEST:
