Re: Notice of Agreement Termination and CDC-RFA-OT21-2103 National Initiative to Address COVID-19
Health Disparities Program Termination,Agreement No. 24-692, supersedes Agreement No. 23-134, Agreement
with Exceptional Parents Unlimited
Dear Lowell,
The County of Fresno (County),through its Department of Public Health(Department),was a recipient of a
Notice of Award from Center for Disease Control and Prevention(Federal),which provided the opportunity for
the Department to contract with local community-based organizations to provide COVID-19 mitigation services
through community health workers. From this opportunity,the Department currently has an agreement with
Exceptional Parents Unlimited, Inc. (EPU)to provide services for Fresno County's Health Disparities Program.
Due to the Department's recent correspondence from the Federal agency, Federal funding is no longer available to
support the Health Disparities Program and termination of all funds was effective as of March 24"', 2025. The
County is providing notice to EPU and invoking Article 6.1 Termination clause under the non-allocation of
funding. This letter will serve as the County's 30-day advance written notice to EPU's Agreement Number 23-
134, amended as Agreement Number 24-692,termination.
The final day of this Agreement shall be April 27"', 2025. Please submit all invoices, along with relevant
supporting documents, under this Agreement term through March 241h, 2025 by April 10", 2025. Any invoices
submitted after April 10"', 2025 will not be eligible for reimbursement due to the funding allocation being
terminated. The final day to access Care Coordination Systems and any other platform provided by the
Department will be April 15"',2025. Additionally, please submit monthly progress reports by April 15", 2025, our
Program staff will be in contact to establish any other close out activity and reporting requirement.
The Department has enjoyed working with EPU under Fresno County's Health Disparities Program and we are
proud of the impact that has been achieved through this partnership. We look forward to other possible
