1 AMENDMENT NO. 3 TO SERVICE AGREEMENT
2 This Amendment No.3 to Service Agreement ("Amendment No. 3") is dated
3 August 20, 2024 and is between each individual Contractor listed in Exhibit A "List of
4 Contractors" attached hereto and incorporated herein by reference, and collectively hereinafter
5 referred to as Contractor(s), and the County of Fresno, a political subdivision of the State of
6 California ("County").
7 Recitals
8 A. On June 20, 2023, the County and the Contractor entered into Specialized Residential
9 Mental Health Services Agreement, effective July 1, 2023, which is County agreement number
10 A-23-283 ("Agreement"), as amended by County agreement number A-23-415, and amended
11 once more by County agreement number A-24-100, for residential mental health services; and
12 B. Contractor(s) has the secured facilities, staff, and expertise, and is licensed by the State
13 of California, to provide residential mental health services, and is willing and able to provide
14 such services to individuals referred by Fresno County pursuant to the terms and conditions of
15 this Agreement; and
16 C. The County now desires to amend the Agreement to add Contractor Central Star
17 Behavioral Health, Inc. and remove Contractor Crestwood Behavioral Health, Inc.
18 D. The parties therefore agree as follows:
19 1. All references to Exhibit A shall be deemed references to Revised Exhibit A-2, which is
20 attached and incorporated by this reference.
21 2. The parties agree that upon execution of this Amendment No. 3, the Agreement is
22 further revised, updated, and amended to add Contractor Central Star Behavioral Health, Inc. Its
23 "Description of Services & Rates" is attached to this Amendment No. 3 as Exhibit B-8.
24 3. When all parties have signed this Amendment No. 3, the Agreement, Amendment No. 1,
25 Amendment No. 2, and this Amendment No. 3 together constitute the Agreement.
26 4. The Contractor represents and warrants to the County that:
27 a. The Contractor is duly authorized and empowered to sign and perform its obligations
28 under this Amendment.

1 b. The individual signing this Amendment on behalf of the Contractor is duly authorized
2 to do so and his or her signature on this Amendment legally binds the Contractor to
3 the terms of this Amendment.
4 5. The parties agree that this Amendment may be executed by electronic signature as
5 provided in this section.
6 a. An "electronic signature" means any symbol or process intended by an individual
7 signing this Amendment to represent their signature, including but not limited to (1) a
8 digital signature; (2) a faxed version of an original handwritten signature; or (3) an
9 electronically scanned and transmitted (for example by PDF document) version of an
10 original handwritten signature.
11 b. Each electronic signature affixed or attached to this Amendment (1) is deemed
12 equivalent to a valid original handwritten signature of the person signing this
13 Amendment for all purposes, including but not limited to evidentiary proof in any
14 administrative or judicial proceeding, and (2) has the same force and effect as the
15 valid original handwritten signature of that person.
16 c. The provisions of this section satisfy the requirements of Civil Code section 1633.5,
17 subdivision (b), in the Uniform Electronic Transaction Act (Civil Code, Division 3, Part
18 2, Title 2.5, beginning with section 1633.1).
19 d. Each party using a digital signature represents that it has undertaken and satisfied
20 the requirements of Government Code section 16.5, subdivision (a), paragraphs (1)
21 through (5), and agrees that each other party may rely upon that representation.
22 e. This Amendment is not conditioned upon the parties conducting the transactions
23 under it by electronic means and either party may sign this Amendment with an
24 original handwritten signature.
25 6. This Amendment may be signed in counterparts, each of which is an original, and all of
26 which together constitute this Amendment.

1 7. The Agreement as amended by this Amendment No. 3 is ratified and continued. All
2 provisions of the Agreement and not amended by this Amendment No. 3 remain in full force and
3 effect. This Amendment No. 3 shall be effective upon execution.

BAKERSFIELD YOUTH PSYCHIATRIC HEALTH FACILITY
DAILY RATE AND ANCILLARY SERVICES FEE SCHEDULE
2024-2025
1. Contracted Daily Rate Services: Psychiatric Health Facility
Service Unit Rate

PHF Administrative Day Da $1,435.88
2. Ancillary Services (not covered in Daily Rate):
• Medications and other pharmacy services
• Mobile Xray
• Laboratory tests
• Ambulance transportation
For clients with Medi-Cal insurance, ancillary services will be directly billed to State
Medi-Cal by providers contracted with CONTRACTOR(Central Star).
For clients with private insurance, ancillary services may be directly billed to private
insurance company by providers contracted with Central Star or will be billed to Central
Star.
For indigent clients, ancillary services will be billed to Central Star by providers
contracted with Central Star.
Fees to COUNTY for ancillary services billed to Central Star and not covered by other
insurance will include the charges to Central Star plus a four percent (4%) processing fee.
