Capacity Funding,Agreement No. 24-586,Agreement with RIZE Consultants,Inc
Dear Nicole,
The County of Fresno(County),through its Department of Public Health(Department),was a recipient of an
award from the California Department of Public Health(State), which provided the opportunity for the
Department to contract with a training consultant to provide health equity consultation and training services.
Through this opportunity,the Department currently has an agreement with RIZE Consultants, Inc. (RIZE)to
provide trainings to Department staff and build equity capacity with subcontracted community-based
organizations. Due to the Department's recent correspondence from the State agency, State funding is no longer
available to support programmatic items and termination of all funds was effective as of March 24'h,2025.The
County is providing notice to RIZE and invoking Article 6.1 Termination clause under the non-allocation of
funding. This letter will serve as the County's 30-day advance written notice to RIZE's Agreement Number 24-
586,termination.
The final day of this Agreement shall be April 28", 2025.Please submit all invoices, along with relevant
supporting documents, under this Agreement term through March 24'1,2025 by April 10'h, 2025. The closeout
invoice for the dates of March 25'h through April 28'11 will follow the submission timeline as stipulated in the
Agreement. Department staff will be in contact to establish any other close out activities and reporting
requirements.
The Department has enjoyed working with RIZE and we are proud of the impact that has been achieved through
this partnership. We look forward to other possible collaborations in the future. Should you have any questions or
