Re: Notice of Agreement Termination and California Department of Public Health's Expansion Laboratory
Capacity Funding,Agreement No. 22-482 with United Way of Fresno and Madera Counties
Dear Stephanie,
The County of Fresno(County),through its Department of Public Health(Department),was a recipient of an
award from the California Department of Public Health's(State), which provided the opportunity for the
Department to contract with local community-based organizations to provide COVID-19 mitigation services
through community health workers (CHWs). Through this opportunity,the Department currently has an
agreement with United Way of Fresno and Madera Counties(UWFM)to provide linkages to Fresno County's
CHW Network through the 211 platform. Due to the Department's recent correspondence from the State agency,
State funding is no longer available to support this effort and termination of all funds was effective as of March
201, 2025. The County is providing notice to UWFM and invoking Article 6.1 Termination clause under the non-
allocation of funding. This letter will serve as the County's 30-day advance written notice to UWFM's
Agreement Number 22-482, termination.
The final day of this Agreement shall be April 28"', 2025. Please submit all invoices, along with relevant
supporting documents, under this Agreement term through March 24', 2025 by April 1 Ot', 2025. The close out
invoice for the dates of March 251''through April 28"will follow the submission timeline as stipulated in the
Agreement. Department staff will be in contact to establish any other close out activities and reporting
requirements.
The Department has enjoyed working with UWFM under Fresno County's CHW Network and we are proud of
the impact that has been achieved through this partnership. We look forward to other possible collaborations in
