Re: Notice of Agreement Conclusion, Agreement Number A-23-213
Dear Mr. Voon,
The County of Fresno (County), through its Department of Public Health (Department), currently has an
agreement with Buddhist Tzu Chi Medical Foundation to provide professional eye exams and
prescription eyeglasses through Fresno County's Rural Mobile Health (RMH) program.
Article eleven (11) of this Agreement indicates a term of May 1, 2023 through April 30, 2025, with an
option to extend the Agreement for an additional twelve (12) month period upon written approval of both
parties no later than the last day of the current term. This letter will serve as the County's advance
written notice that the agreed upon term of the above referenced Agreement is set to end on April 30,
2025 and will not be extended. The Department has enjoyed working in collaboration with Buddhist
Tzu Chi Medical Foundation to provide RMH services to Fresno County communities and encourages
your team to continue to maximize RMH services through the end of the Agreement term.
The final day of this Agreement will be April 30, 2025. Any services provided, or expenses incurred
after this date cannot be billed to this Agreement. Please submit any outstanding invoices or final
reports no later than May 30, 2025.
