1 AMENDMENT IV TO AGREEMENT
2 THIS AMENDMENT, hereinafter referred to as Amendment IV, is made and entered into this
3 3rd day of December 12024, by and between the COUNTY OF FRESNO, a Political
4 Subdivision of the State of California, hereinafter referred to as "COUNTY", and each CONTRACTOR to
5 be listed in Revised Exhibit A-3 "List of Contractors," collectively hereinafter referred to as
6 "CONTRACTORS". Reference in this Agreement to party or"parties" shall be understood to refer to
7 COUNTY and each individual CONTRACTOR(S), unless otherwise specified.
8 WITNESSETH:
9 WHEREAS, COUNTY through its Department of Behavioral Health (DBH) has a need for licensed
10 psychiatrists in order to provide psychiatric services to patient/clients, as well as additional services as
11 stated in the Agreement; and
12 WHEREAS, the parties entered into that certain Agreement, identified as COUNTY Agreement No.
13 19-200, effective May 7, 2019, Amendment I, identified as COUNTY Agreement No. 23-081, effective
14 February 28, 2023, Amendment II, identified as COUNTY Agreement No. 23-305, effective June 20,
15 2023, and Amendment III, identified as COUNTY Agreement No. 23-591, effective November 7, 2023
16 Amendment III, collectively referred to as the "Agreement,"whereby CONTRACTORS are qualified and
17 willing to source, screen, present, and coordinate certain logistics for locum tenens psychiatrists; and
18 WHEREAS, the COUNTY now desires to revise the Rates of Reimbursement for CONTRACTOR
19 Jackson & Coker LocumTenens, LLC to meet current market rates for locum tenens psychiatry services
20 and to maintain an adequate provider-to-service recipient ratio by retaining psychiatry providers provided
21 through this agreement; and
22 NOW, THEREFORE, for good and valuable consideration, the receipt and adequacy of which is
23 hereby acknowledged, the parties agree as follows:
24 1. That all references to "Revised Exhibit A-2" shall be deemed references to "Revised
25 Exhibit A-3", which is attached and incorporated by this reference.
26 2. The CONTRACTORS' Exhibit B sub-parts, "Rates of Reimbursement," as indicated on
27 Revised Exhibit A-3, are attached and incorporated by this reference.
28 3. That all references to "Exhibit B-1A" shall be deemed references to "Exhibit B-1 B", which

1 is attached and incorporated by this reference.
2 4. CONTRACTORS hereby agree to all terms of the Agreement, as amended, and agree to
3 be bound by the terms of the Agreement, as amended. CONTRACTORS hereby acknowledge that
4 they have received a complete copy of the Agreement, as amended.
5 5. The parties agree that this Amendment IV is sufficient to amend the Agreement; and that
6 upon execution of the Agreement, Amendment I, Amendment II, Amendment III, and Amendment IV
7 together shall be considered the Agreement.
8 6. The Agreement, as hereby amended, is ratified and continued. All provisions, terms,
9 covenants, conditions, and promises contained in the Agreement and not amended herein shall remain in
10 full force and effect. This Amendment IV shall be effective upon execution.

Locum Tenens Services
Rates of Reimbursement

SERVICE TYPE RATES
Dail All-inclusive Rate— Psychiatrist
This rate is for services provided between 8:OOam- Psychiatrist: $ 300-375 /hr
5:OOpm Monday through Sunday, with a 40-hour Nurse Practitioner: $ 185-220 /hr
maximum per week.
Dail All-inclusive Rate— Psychiatrist & Nurse Practitioner Telehealth
This rate is for services provided between 8:OOam- Psychiatrist: $ 230-276 /hr
5:OOpm Monday through Sunday, with a 40-hour Nurse Practitioner: $ 208 /hr
maximum per week. The rate for NP supervision is for NP Supervision: $ 325 /hr
1 hour per 1 FTE per week.
Dail All-inclusive Rate - Forensic Services
This rate is for services provided between 8:OOam- Psychiatrist: $ 300-395
5:OOpm Monday through Sunday with a 40-hour Nurse Practitioner: $ N/A
maximum per week, with an emphasis on services
performed in the County Jail or at trial evaluations.
Weekend Call Rate
This rate is for carrying a pager and taking calls via Psychiatrist: $ 1,200-1,500 /hr
phone from Saturday at 5:OOpm through Monday at Nurse Practitioner: $ N/A
8:OOam, when the psychiatrist is scheduled less than
eight hours on Saturday or Sunday.
In-House Call Rate
This rate is for carrying a pager and taking calls via Psychiatrist: $ 1,650-2,062.50
phone from Saturday at 5:OOpm through Monday at /hr
8:OOam, when the psychiatrist is scheduled to work an Nurse Practitioner: $ N/A
eight-hour shift on Saturday or Sunday between
8:OOam and 5:OOpm.
Weeknight Call Rate
This rate is for carrying a pager and taking call via Psychiatrist: $ 450-562.50 /hr
phone from Monday at 5:OOpm through Saturday at Nurse Practitioner: $ N/A
8:OOam, no matter whether the psychiatrist is
scheduled to work a shift Monday through Friday.
Overtime Rate
This rate is for hours worked after 40 hours per week Psychiatrist: $ 450-562.50 /hr
or in the instance the psychiatrist is called back into Nurse Practitioner: $ N/A
the facility while on call.
Acquisition Fee
This rate is what the County will pay to the Contractor Psychiatrist: $ 45,000
in the event a psychiatrist is hired and placed as a Nurse Practitioner: $ N/A
permanent staff psychiatrist by the County.
