HomeMy WebLinkAbout2307M CCO 05.pdf COUNTY OF FRESNO CONSTRUCTION MANAGEMENT CONTRACT CHANGE ORDER ROAD MAINTENANCE AND REHABILITATION CHANGE ORDER NO. 5 ACCOUNT (SB-1)ASPHALT CONCRETE OVERLAYS OCTOBER 9, 2024 VARIOUS LOCATIONS IN FRESNO COUNTY PAGE 1 OF 4 CONTRACT NO. 23-07-M TO: YARBS GRADING AND PAVING, INC: Make the following changes to the plans and specifications or do work not in the plans and specifications for this contract. Because of the change(s), 44 -WORKING DAYS will be added to the contract time. The revised completion date was September 17, 2024. PART I: RAISE DRAIN INLETS AT LOCATION NO. 3, BRAWLEY AVENUE - EXTRA WORK This extra work was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications, raise two (2)— 36-inch drain inlets to match the existing grade at Location No. 3, Brawley Ave, on the northeast corner and northwest corner of Brawley and Belmont Avenues. Perform all work in accordance with Section 71-5, "Adjust Drainage Structures," of the Standard Specifications, as directed by the engineer. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope," of the Special Provisions. Total compensation for this work resulted in $8,804.44. This amount will be paid through Bid Item No. 3, Supplemental Work, with no net increase to the contract. A total of six (6) additional working days will be granted for this work. TOTAL COST PART I: $0.00 (PAID IN BID ITEM NO. 3) TOTAL TIME EXTENSION PART I: SIX(6) -WORKING DAYS PART II: PLACE ADDITIONAL TYPE E DIKE AND DOWN-DRAINS AT LOCATION NO. 11A, CSA 35 'AS' DOGWOOD - EXTRA WORK This extra work was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications," place an additional 2,070 LF Type E dike, and a total of four (4) down-drains at Location No. 11A, CSA 35 'AS' Dogwood, using hot mix asphalt (HMA). Perform all work in accordance with Section 39-2.01 C(9), "Miscellaneous Areas, Dikes, and Berms," of the Special Provisions, as directed by the engineer. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope," of the Special Provisions. Total compensation for this work resulted in a $15,057.76 increase. This amount will be paid as extra work. This work did not impact the critical path for the project, therefore, granting additional contract time was not warranted. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications," place an additional 1,750 LF Type E dike at Location No. 12A, CSA 35 `AG' Wild Rose, using HMA. Perform all work in accordance with Section 39-2.01 C(9), "Miscellaneous Areas, Dikes, and Berms," of the Special Provisions, as directed by the engineer. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope," of the Special Provisions. Total compensation for this work resulted in a $15,057.76 increase. This amount will be paid as extra work. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications, install a total of 14 survey monuments frames and covers (G5 boxes) on Elkhorn Avenue between Elm and Jameson Avenues. Perform all work in accordance with Section 78.2, "Survey Monuments," of the Special Provisions and Standard Specifications. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope," of the Special Provisions. Total compensation for this work resulted in $30,967.06. This amount will be paid through Bid Item No. 3, Supplemental Work, with no net increase to the contract. A total of 33 additional working days will be granted for this work. TOTAL COST PART IV: $0.00 (PAID IN BID ITEM NO. 3) TOTAL TIME EXTENSION PART IV: 33 -WORKING DAYS PART V: INSTALL A TRAFFIC SENSOR LOOP AT LOCATION NO. 6, ASHLAN AVENUE - EXTRA WORK This extra work was requested by the engineer. Perform all work in accordance with Section 87-1.03V(2), "Inductive Loop Detectors,"of the Revised Standard Specifications Dated September 2, 2016, as directed by the engineer. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope," of the Special Provisions. Total compensation for this work resulted in $5,535.41. This amount will be paid through Bid Item No. 3, Supplemental Work, with no net increase to the contract. A total of 11 additional working days will be granted for this work.
