RE: One-Year Extension of Agreement No. P-20-382
Dear Dr. Kenworthy:
Agreement No. P-20-382, which was approved by Purchasing on October 1, 2020, allows for
the option of a one-year extension upon written approval of both parties no later than thirty days
prior to the first day of the next twelve-month extension period (July 1, 2024). The Department
of Behavioral Health Director, or designee, is authorized to execute such written approval on
behalf of the County based on the provider's satisfactory performance.
I have reviewed the performance of K&L Clinical Forensic Practice in providing the contracted
services and find it continues to be satisfactory. Therefore, I would like to extend the agreement
from July 1, 2024 through June 30, 2025. If you, as the signing authority for K&L Clinical
Forensic Practice, would also like the agreement to be extended as stated, please sign and date
on the line below and return this letter by May 31, 2024 to Emma Mejia at
emmejia@fresnocountyca.gov.
