RE: One-Year Extension of Agreement No. A-23-282
Dear Ms. Zavala:
Agreement No. 23-282, approved by the Board of Supervisors on June 20, 2023, allows
for the option of a one-year extension upon written approval of both parties and
authorized by the Director to execute such written approval on behalf of the County.
Department staff have reviewed the performance of California Psychological Institute,
Inc.'s CWMH program in providing the contracted services and find it continues to be
satisfactory. Therefore,we would like to extend the agreement for Fiscal Year 2024-
2025, July 1, 2024 through June 30, 2025. If you, as the signing authority for California
Psychological Institute, Inc., would also like the agreement to be extended as stated,
please sign and date on the line below and e-mail a copy of the signed letter to Huong
Chung, Staff Analyst at hchung@,fresnocoLjnjyca,aQy,
