RE: One-Year Extension of Agreement No. P-23-486
Dear Dov Schechter:
Agreement No. P-23-486, which was approved on September 13, 2023, allowed for the option
of four one-year extensions upon written approval of both parties no later than thirty days prior
to the first day of the next twelve-month extensive period, July 1, 2024, and authorized the
Director to execute such written approval on behalf of the County.
I have reviewed the performance of Streamline in providing queries for the Death Master file
review and find it continues to be satisfactory. Therefore, I would like to extend the agreement
from July 1, 2024 through June 30, 2025. If you, as the signing authority for Streamline, would
also like the agreement to be extended as stated, please sign and date on the line below and
return this letter by June 30, 2024 to:
