COUNTY OF FRESNO CONSTRUCTION MANAGEMENT CONTRACT CHANGE ORDER GOLDEN STATE BOULEVARD PHASE 1 CHANGE ORDER NO. 24 AMERICAN AVENUE TO MISSION STREET MARCH 21,2024 CONTRACT NO, 22-14-C PAGE 2 OF 4 PART Ik CONSTRUCT HOT MIX ASPHALT DOWN-DRAINS IN KINGSBURG REGION—EXTRA WORK This extra work was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work,"of the Standard Specifications, construct hot mix asphalt down-drains under the metal beam guard railing(northbound), between approximate sta. 118+00 and 122+00, in the Kingsburg region. Perform all work according to Section 39-2.01 C(9), "Miscellaneous Areas, Dikes, & Berms,"of the Special Provisions, as directed by the engineer. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope,"of the Special Provisions.Total compensation for this work resulted in $5,831.19, this amount will be paid as extra work. This extra work did not impact the critical path for the project, therefore, granting additional contract time was not warranted. TOTAL COST PART II: $6,831.19 TOTAL TIME EXTENSION PART II: ZERO(0)-WORKING DAYS PART III: RE-GRADE DIRT MEDIAN ALONG THE NEWLY CONSTRUCTED LEFT TURN POCKET AT THE SOUTH VAN HORN AVENUE INTERSECTION TO ENHANCE DRAINAGE-EXTRA WORK This change was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work,"of the Standard Specifications, re-grade the dirt median along the left turn pocket at the South Van Horn Avenue intersection (Selma region)to the grades shown on the attached revised Plan Sheet 72B (replace plan sheet 72A with revised sheet 72B). Perform all work according to Section 19, "Earthwork,"of the Standard Specifications and/or the Special Provisions. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope,"of the Special Provisions. Total compensation for this work resulted in $4,721.55, this amount will be paid as extra work. This extra work did not impact the critical path for the project, therefore, granting additional contract time was not warranted. COUNTY OF FRESNO CONSTRUCTION MANAGEMENT CONTRACT CHANGE ORDER GOLDEN STATE BOULEVARD PHASE I CHANGE ORDER NO. 24 AMERICAN AVENUE TO MISSION STREET MARCH 21, 2024 CONTRACT NO. 22-14-C PAGE 3 OF 4 In accordance with Section 4-1,05, "Changes and Extra Work,"of the Standard Specifications,extend the driveway approach to the parking lot near the I"Street and Whitson Street intersection, on the eastside, by approximately 3 feet to flatten the driveway slope. This work includes saw cutting the existing pavement at the new match point, removing and disposing the asphalt pavement, re-grading and compacting the subgrade, and paving back the driveway approach using Type A Y-Inch Hot Mix Asphalt. Perform all work according to various applicable sections of the Standard Specifications and/or Special Provisions. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope,"of the Special Provisions.Total compensation for this work resulted in$2,732.56,this amount will be paid as extra work. This extra work did not impact the critical path for the project,therefore, granting additional contract time was not warranted. TOTAL COST PART IV: $2,732.56 INCREASE TOTAL TIME EXTENSION PART IV: ZERO(0) -WORKING DAYS PART V: SAWCUT EXISTING ASPHALT PAVEMENT AT THE RIGHT-OF-WAY,ALONG AT THE PARKING LOT AND PREPARE SUBGRADE BETWEEN McCALL AVENUE AND TODD STREET-EXTRA WORK This extra work was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work,"of the Standard Specifications, sawcut existing asphalt pavement at the match point in parking area and prepare subgrade to pave back the area behind the newly constructed concrete curb. Remove and dispose the old asphalt pavement. Perform all work according to various applicable sections of the Standard Specifications and/or Special Provisions. Compensation for this work will be made as extra work at force account according to Section 9-1.03, "Payment Scope,"of the Special Provisions. Total compensation for this work resulted in $3,535.98,this amount will be paid as extra work. This extra work did not impact the critical path for the project,therefore, granting additional contract time was not warranted. This work includes repairing several irrigation sprinklers, grading and storing lawn, and replacing the damaged shrubs. Perform all work according to Sections 20, "Landscape,"of the Standard Specifications and/or Special Provisions. Compensation for this work will be made at an agreed lumpsum price of$3,080 according to Section 9-1.03, "Payment Scope,"of the Special Provisions. This amount includes compensation for all labor, equipment, materials, incidentals, markups, and profit for the above referenced work, no additional compensation will be made. This amount will be paid as extra work. This extra work did not impact the critical path for the project, therefore, granting additional contract time was not warranted.
