The County of Fresno desires to extend this contract for an additional three-month period effective April 1, 2024 through June 30, 2024. Prices will be firm for the contract period. Amendment provisions shall remain in full force and effect with any contract renewals. Maximum: The County of Fresno desires to increase this Agreement in the amount of thirty-one thousand five hundred dollars ($31,500.00). In no event shall services performed and/or fees paid under this Agreement be in excess of one hundred thirty-one thousand five hundred dollars ($131,500.00). Electronic Signatures. The parties agree that this Agreement may be executed by electronic signature as provided in this section. A. An "electronic signature" means any symbol or process intended by an individual signing this Agreement to represent their signature, including but not limited to (1) a digital signature; (2) a faxed version of an original handwritten signature; or(3) an electronically scanned and transmitted (for example by PDF document) of a handwritten signature. B. Each electronic signature affixed or attached to this Agreement (1) is deemed equivalent to a valid original handwritten signature of the person signing this Agreement for all purposes, including but not limited to evidentiary proof in any administrative or judicial proceeding, and (2) has the same force and effect as the valid original handwritten signature of that person. C. The provisions of this section satisfy the requirements of Civil Code section 1633.5, subdivision (b), in the Uniform Electronic Transaction Act(Civil Code, Division 3, Part 2, Title 2.5, beginning with section 1633.1). D. Each party using a digital signature represents that it has undertaken and satisfied the requirements of Government Code section 16.5, subdivision (a), paragraphs (1)through (5), and agrees that each other party may rely upon that representation. This Agreement is not conditioned upon the parties conducting the transactions under it by electronic means and either party may sign this Agreement with an original handwritten signature. Fresno County MCH Needs Assessment I Facente Consulting CONTRACT NO.P-21-250 Revised Attachment"A" Facente Consulting,LLC Page 2 of 6 03/27/2024 ACTIVITIES Facente Consulting is excited to support Fresno County Department of Public Health's Maternal and Child Health Program with their current needs assessment. Facente Consulting will assemble an appropriate project team from our pool of expert consultants to provide consulting services for the needs assessment for the extended term from April 1,2024 to June 30, 2024. Phase 1: Desk Review and Data Collection (April—May 2024) • Review existing documents(e.g., past needs assessments and plans, notes from MCH internal meetings and stakeholder meetings,goals and priorities documents) • Conduct three virtual focus groups. Facente Consulting would develop a recruitment flyer/email, schedule the groups, contact the participants,conduct the groups, and provide incentives.We would need assistance from the County with recruitment,either by putting us directly in touch with potential participants or introducing us to organizations that could recruit from their clients/community.These focus groups could be with any priority population that Fresno believes is important, but given current domains and data gaps,we recommend: o Women/Maternal Health—Spanish o Special Needs Children Parents/Caregivers—English o Adolescents—English • Implement two surveys.One survey would be widely distributed to key stakeholders identified by the County,to get additional input on priority needs in the five domains(Women/Maternal Health, Perinatal,Children,Adolescents).The other survey would focus on the fifth domain (Special Needs) and be geared towards understanding system capacity to meet the needs of this group. Phase 2: Data Analysis and Report (May—June 2024) • Analyze survey and focus group data and prepare report. Focus group data will be analyzed using thematic analysis techniques,survey data will be analyzed in a way that helps inform priorities,and a final summary report will be prepared. • Complete needs assessment packet. Facente Consulting will complete the needs assessment packet on behalf of the County.This will likely involve some meetings with MCH to ensure we are inclusive and accurate with the content. Phase 3: Plan for Tracking Progress (optional: June 2024) • Develop a system for tracking priority needs and action items.We will work with MCH to determine how best to track implementation of the needs assessment plan, including timeline for regular updates on progress. • Develop tracking tools.We will develop simple tracking tools(e.g., Excel spreadsheet, meeting agendas) that MCH can use to document their progress on the plan.
