Re: One-Year Extension of Agreement# P-20-380
Dear Ms. Bolano,
The County's Agreement# P-20-380 which was approved by the Board of Supervisors on June
30, 2020, allowed for the option of two - twelve (12) month renewals upon satisfactory
performance of services during the then-current period (July 1, 2020 through June 30, 2023),
and by written notice of renewal by the Department of Public Health (Department) Director.
Contract service performance was reviewed by the Department and was determined to be
satisfactory. Therefore, the Department provides this letter as notice of renewal, extending the
Agreement for an additional twelve (12) month period beyond the current term, July 1, 2024
through June 30, 2025. If you, as the signing authority for Saint Agnes Medical Center, would
also like to extend the agreement as stated, please sign and date below and return this letter to
Douglas Loera at County of Fresno, Department of Public Health - 1221 Fulton Street, Fresno,
CA 93721, or via email at dloera _fresnocountyca.gov by April 2, 2024.
Please note, a renewal letter signed and dated, past April 2, 2024, is outside of the contract's
renewal condition clause and will make the renewal letter invalid, causing a lapse in the contract
term. A lapsed contract would require Board approval, which could result in a lapse in service
and compensation, depending on processing timelines and/or cancellation of service contract,
pending Board action. Your attention to this request is greatly appreciated to avoid a lapse in
the contract term.
