RE: One-Year Extension of Agreement No. A-23-298
Dear Joyce Fields-Keene:
County of Fresno Agreement No. A-23-298, which was approved by the Board of Supervisors on June 20, 2023 allowed
for the option of one (1) twelve-month extension upon written approval of both parties no later than sixty days prior to the
first day of the next twelve-month term, which would begin July, 1, 2024, and authorized the Director or designee of the
Department of Behavioral Health to execute such written approval on behalf of the County.
The Department finds that Central California Faculty Medical Group, Inc. is in good standing and continues to provide
satisfactory services and would like to extend the agreement from July 1, 2024 through June 30, 2025. If you, as the
signing authority for Central California Faculty Medical Group, Inc. would also like the agreement to be extended as stated,
please sign and date on the line below and return this letter by April 30, 2024 to:
