2 THIS SECOND AMENDMENT TO AGREEMENT ("Amendment") is made and entered into this
3 9th day of October, 2018, by and between COUNTY OF FRESNO, a Political Subdivision of the State
4 of California, Fresno, California ("COUNTY"}, and Applied Polygraph, LLC, a California limited liability
5 company, whose address is 9454 Wilshire Boulevard, Suite 600, Beverly Hills, California
6 ("CONTRACTOR").
7 WITNESS ETH:
8 WHEREAS, COUNTY and CONTRACTOR entered into Agreement number P-16-464-T, dated
9 July 13, 2016 ( "Agreement"), pursuant to which CONTRACTOR agreed to provide employment
10 testing services to COUNTY; and
11 WHEREAS, on September 1, 2018, a First Amendment to the Agreement was executed,
12 increasing the contract maximum to $99,999 for polygraph services; and
13 WHEREAS, COUNTY and CONTRACTOR again desire to amend the Agreement in order to
14 increase the maximum compensation amount and extend the term of the Agreement.
15 NOW, THEREFORE, for good and valuable consideration, the receipt and adequacy of which
16 is hereby acknowledged, COUNTY and CONTRACTOR agree as follows:
17 1.Section 2 of the Agreement, located on page 2, is deleted in its entirety and replaced
18 with the following:
19 "This Agreement shall become effective on July 1, 2016 and shall terminate on January
20 14, 2019, with two (2) optional one (1) year renewals, unless earlier terminated as set forth in Section
21 4 of this Agreement."
22 2.Section 3 of the Agreement, located on page 2 and 3, is deleted in its entirety and
23 replaced with the following:
24 "CONTRACTOR's sole compensation for performance of the services rendered
25 pursuant to this Agreement, including travel costs, will be a fee of one hundred seventy five dollars
26 ($175) per Polygraph Examination for the period of July 1, 2016 to January 14, 2017, one hundred
27 ninety dollars ($190) per Polygraph Examination for the period of January 15, 2017 to January 14,
28 2018, and two hundred dollars ($200) per Polygraph Examination for the period of January 15, 2018 to

1 IN WITNESS WHEREOF, the parties hereto have executed this Second Amendment to
2 Agreement as of the day and year first hereinabove written.
