Home My WebLink AboutAgreement A-24-021 Amendment No. 3 to Master Agreement.pdf Agreement No. 24-021 1 AMENDMENT NO. 2 TO SERVICE AGREEMENT 2 This Amendment No. 2 is dated January 9, 2024 and is by and between the 3 COUNTY OF FRESNO, a Political Subdivision of the State of California, Fresno, California 4 ("COUNTY"), and each Provider listed in Revised Exhibit A to this Amendment (collectively 5 referred to as "PROVIDER"). Reference in this Agreement to "party" or "parties" shall be 6 understood to refer to COUNTY and each PROVIDER, unless otherwise specified. 7 Recitals 8 A. COUNTY, through its Department of Behavioral Health, is a Mental health Plan as 9 defined in Title 9 of the California Code of Regulations (C.C.R.), section 1810.226 and 10 COUNTY, through its Mental Health Plan, is in need of providers to provide specialty mental 11 health services to certain COUNTY's Medi-Cal beneficiaries; and 12 B. On June 23, 2020, the COUNTY and the PROVIDER entered into Agreement No. 20- 13 236, amended by Agreement No. 23-271 (collectively referred to as the "Agreement") where in 14 PROVIDERS agreed to provide Specialty Mental Health Services to certain COUNTY's Medi- 15 Cal beneficiaries. 16 C. PROVIDER Victoria Montufar incorporated her business on May 30, 2023, La Familia 17 Therapy, and will operate under her new business name La Familia Therapy Services, Inc.. 18 D. The COUNTY now desires to remove PROVIDER Victoria Montufar from the Agreement 19 and add the following PROVIDERS to the Agreement: (1) La Familia Therapy Services, Inc. and 20 (2) Ka Bao Vang. 21 The parties therefore agree as follows: 22 1. All references to Revised Exhibit A shall be deemed references to Revised 23 Exhibit A-2, which is attached and incorporated by this reference. 24 2. The parties agree that upon execution of this Amendment No. 2, Agreement No. 25 20-236, as previously amended, is further revised, updated and amended to remove 26 PROVIDER Victoria Montufar and to add PROVIDERS: (1) La Familia Therapy Services, Inc. 27 and (2) Ka Bao Vang. 28 3. COUNTY and PROVIDER agree that this Amendment is sufficient to amend the 1 1 Agreement and, that upon execution of this Amendment No. 2, the Agreement, Amendment No. 2 1, and Amendment No. 2 together shall be considered the Agreement. 3 4. The Agreement, as hereby amended, is ratified, and continued.
