AMENDMENT. The Parties amend the Agreement as follows (complete each as indicated below):
1. RECITALS. Other Party and FCSS are referred to separately as a “Party” and collectively as the
“Parties”. The Parties entered into the above-referenced Agreement and hereby desire to amend said
Agreement as set forth below.

4. OTHER CHANGES (leave blank if none): Contract Payment Schedule Change from Payment Schedule 1 –
Fixed installment payment to Payment Schedule 4 – Other: The “Contract Amount” of not to exceed
$599,902.00, each invoice for payment of which Contractor shall submit to FCSS quarterly and in
accordance with Article 2, and the payment of each invoice for which FCSS shall make to Contractor
within 30 days after: (A) Contractor has completed, in accordance with this Agreement, the Services
required of Contractor for the period for which Contractor requests payment; and (B) FCSS has received
and approved Contractor’s invoice describing, at a minimum, the Services that Contractor completed,
date(s) on which such Services were completed, and the payment requested.
Contractor agrees to comply with our requirements set forth in the First 5 Fresno County Service Provider
Manual 2016/2018, which is incorporated herein and available at the following link:
