AMENDMENT IV TO AGREEMENT 1
2 THIS AMENDMENT is made and entered into this 17th day of April, 2018, by
3 and between the COUNTY OF FRESNO, a political subdivision of the State of California, hereinafter
4 referred to as "COUNTY", and BAART BEHAVIORAL HEALTH, INC., a California Corporation, whose
5 remit to and mailing address is 1720 Lakepointe Drive, Suite 117, Lewisville, TX 75057 hereinafter
6 referred to as "PROV! DER."
7 WHEREAS, the parties entered into that certain Agreement, identified as COUNTY Agreement
8 No. 13-492, effective July 1st, 2013, as amended by Amendment I, identified as COUNTY Agreement
9 No. 13-492-1 effective September 23, 2014, as amended by Amendment 11, identified as COUNTY
10 Agreement No. 13-492-2 effective July, 1, 2015, and as amended by Amendment 111, identified as
11 COUNTY Agreement No. 13-492-3 effective June 20, 2017, hereafter referred to collectively as the
· 12 Agreement; and
13 WHEREAS the parties desire to amend the Agreement, regarding changes as stated below and
14 restate the Agreement in its entirety.
15 NOW, THEREFORE, in consideration of their mutual promises, covenants and conditions,

hereinafter set forth, the sufficiency of which is acknowledged, the parties agree as follows:
1. That the following text in the Agreement 13-492, Page Four (4), Section 4.D,
18 beginning at Line Fourteen (14), with the word "In" and ending on Page Four (4), Line Sixteen (16) with the
19 word "Agreement" be deleted and the following inserted in its place:
20 D.
21 1) Fiscal Years 2013-14 through 2014-15 -In no event shall the total
22 compensation for actual services performed under this Agreement be in excess of Eighty-Four Thousand
23 and No/100 Dollars ($84,000.00) for each twelve (12) month period from July, I, 2013 through June 30,
24 2015.
25 2) Fiscal Years 2015-16 through 2016-17 -In no event shall the total
26 compensation for actual services performed under this Agreement be in excess of Ninety-Five Thousand
27 and No/100 Dollars ($95,000.00) for each twelve (12) month period from July 1, 2015 through June 30,
28 2017.
1 IN WITNESS WHEREOF, the parties hereto have executed this Amendment IV as of the day and
2 year first hereinabove written .
