3 THIS AGREEMENT is made and entered into this 12th day of September, 2017, by
4 and between the COUNTY OF FRESNO, a Political Subdivision of the State of California,
5 hereinafter referred to as "COUNTY", and The Behavioral Tech, LLC, a Washington State Limited
6 Liability Company whose address is 1107 NE 45th Street, Suite 230, Seattle, WA 98105,
7 hereinafter referred to as "CONTRACTOR".

W I T N E 5 5 E T H:
WHEREAS, COUNTY, through its Department of Behavioral Health (DBH) is in need
1 0 of a qualified agency to provide training and related supplies for the Dialectical Behavior Therapy
11 Intensive/Foundational Training Part 1 & Part 2; and
12 WHEREAS, CONTRACTOR is qualified and willing to provide DBH clinical staff with
13 Dialectical Behavior Therapy (DBT) training, a cognitive behavioral treatment which emphasizes
14 individual psychotherapy and group skills training to help people learn and use new skills and
15 strategies to develop a life that they experience as worth living, pursuant to the terms and
16 conditions of this Agreement.
17 NOW, THEREFORE, in consideration of the mutual covenants, terms and conditions
18 herein contained, the parties hereto agree as follows:
19 1. OBLIGATIONS OF THE CONTRACTOR
20 CONTRACTOR shall provide Dialectical Behavior Therapy (DBT)
21 Intensive/Foundation Training consisting of Part I and Part II and consultation as described in the
22 Scope of Work, in Exhibit A, as set forth hereto and by this reference incorporated herein and
23 made part of this Agreement.
24 CONTRACTOR shall also provide Team Consultation to DBH staff via
25 phone/video to help with the implementation of DBT in the work setting .
26 CONTRACTOR shall provide participants of this training with credit for Continuing
27 Education hours.

1 IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the

day and year first hereinabove written.
