THIRD AMENDMENT TO AGREEMENT
THIS THIRD AMENDMENT TO AGREEMENT, ("Third Amendment")is made and
entered into this 26th day of September 2017, by and between the COUNTY OF FRESNO, a
political subdivision of the State of California,("COUNTY") and Evelyn Machado, M.S., dba
ALTERNATIVE SENTENCING CHOICES, whose address is PO Box 63, Fresno, CA
93707,("CONTRACTOR").
WITNESSETH:
WHEREAS, COUNTY and CONTRACTOR entered into Procurement Agreement No. P-
15-431-R, dated November 2, 2015 (hereinafter "Agreement") pursuant to which CONTRACTOR
agreed to provide alternative sentencing services as needed by COUNTY's Public Defender; and
WHEREAS COUNTY and CONTRACTOR entered into an Amendment Number One to
the Agreement ("First Amendment") dated November 23, 2016, extending the original 1 year term
of the Agreement for an additional year; and
WHEREAS COUNTY and CONTRACTOR entered into Amendment Number Two, dated
May 6, 2017,increasing the maximum compensation under the Agreement to $99,999; and
WHEREAS COUNTY and CONTRACTOR now desire to amend the Agreement to further
increase the maximum compensation under the Agreement so that CONTRACTOR may be fully
compensated for unanticipated, but required additional services performed by CONTRACTOR
during the extended contract term ending on November 1, 2017, which contract maximum
exceeds Purchasing's contractual authority.
NOW, THEREFORE, COUNTY and CONTRACTOR agree as follows:
The MAXIMUM Section of the Agreement, set forth on Page one (1) of the original
Procurement Agreement, as amended is deleted in its entirety and replaced with the following:
"MAXIMUM:
In no event shall services performed under this Agreement be in excess of one hundred
and fifteen thousand dollars ($115,000) for the term of this Agreement, inclusive of renewal
periods."
COUNTY and CONTRACTOR agree that this this Third Amendment shall be effective as

of May 1, 2017.
IN WITNESS WHEREOF, the parties hereto have executed this Third Amendment
as of the day and year first hereinabove written.
