AMENDMENT IV TO AGREEMENT 12-253
THIS AMENDMENT IV TO AGREEMENT (hereinafter "Amendment IV") is made
and entered into this 22"d day of August, 2017, by and between COUNTY OF FRESNO, a
Political Subdivision of the State of California, Fresno, California (hereinafter
"COUNTY"), and Risico, Inc., whose address is 2425 W Shaw Ave , Fresno, CA 93711
(hereinafter "CONTRACTOR").
WITNESSETH:
WHEREAS, COUNTY and CONTRACTOR entered into Agreement number 12-253,
effective October 1, 2012, which the parties thereafter amended by executing Amendment I
effective April1, 2014, Amendment II effective July 29, 2014 and Amendment Ill effective
October 27, 2015 (all collectively referred to as the "Agreement), pursuant to which
CONTRACTOR agreed to provide workers' compensation claims administration and selected
ancillary services to COUNTY; and
WHEREAS, COUNTY and CONTRACTOR now desire to amend the Agreement in order to
enter into a ninety (90) day extension of the current contract that is set to expire on September
30, 2017.
NOW, THEREFORE for good and valuable consideration, the receipt and adequacy of
which is hereby acknowledged, COUNTY and CONTRACTOR agree as follows :
1) The Term of the Agreement, set forth in Section Two {2) of the Agreement,
shall be extended an additional three months, terminating December 31, 2017.
