Re: Notice of Termination, Contract Number: A-22-457
Dear Ms. Jackson:
This letter serves as written notice from the County of Fresno to confirm termination of the Service
Agreement (A-22-457) between the County of Fresno Department of Public Health and Dutchess of
Doula.
Pursuant to Article 5 of this Agreement, please accept this letter as advance written notice of our
intention to terminate the above-referenced Agreement. The effective date of termination shall be
October 15, 2023. Pursuant to Article 2.3 of this Agreement, Dutchess of Doula received an advance
payment in the amount of$56,500 and currently has $11,004.76 remaining to be reconciled. Pursuant
to Article 5.5 of this Agreement, the County may demand repayment by the Contractor of any monies
disbursed to the Contractor under this Agreement that, in the County's sole judgement, were not
expended in compliance with this Agreement. The Contractor shall promptly refund all such monies
upon demand, or the remaining monies may be subtracted from the final invoice.
The Department of Public Health has enjoyed working in collaboration with Dutchess of Doula.
