HomeMy WebLinkAboutAgreement A-11-318-1 with CIBHS.pdf12345678910111213141516171819202122232425262728AMENDMENTITOAGREEMENTTHISAMENDMENTismadeandenteredintothisdayof,2016,byandbetweentheCOUNTYOFFRESNO,apoliticalsubdivisionoftheStateofCalifornia,hereinafterreferredtoas"COUNTY",andCALIFORNIAINSTITUTEFORBEHAVIORALHEALTHSERVICES,formerly,CALIFORNIAINSTITUTEFORMENTALHEALTH,aCaliforniaNon-ProfitOrganization,whoseaddressis212519thStreet,Sacramento,California,95818,hereinafterreferredtoas“CONTRACTOR”.WHEREAS,COUNTY,throughitsDepartmentofBehavioralHealth(DBH),requiressystemtransformationconsultationservicesandtrainingforCOUNTYstaff,contractedproviderstaff,consumersandfamilymembers,andotherinterestedcommunitymemberswithinFresnoCountyinaccordancewiththeMentalHealthServicesAct(MHSA)Stateapprovedplan;andWHEREAS,CONTRACTORhastheexperience,expertiseandstafftoprovidesystemtransformationconsultationandtrainingrequiredforDrugMedi-CalOrganizedDeliverySystemwaiverandMHSAimplementationandsystemtransformationactivities.NOW,THEREFORE,inconsiderationoftheirmutualpromises,covenantsandconditions,thepartiesheretoagreeasfollows:1.ThatthefollowingtextintheAgreement,PageOne(1),beginningwithParagraphOne(1),LineTwenty-Four(24),withtheletter“C”andendingonPageOne(1)LineTwenty-Five(25)withtheword“herein”bedeletedandthefollowinginsertedinitsplace:“C.Suchconsultationand/ortrainingprogramsareprovisionallyidentifiedinExhibitA-l,attachedheretoandbythisreferenceincorporatedherein.”2.ThatthefollowingtextintheAgreement,PageFour(4),beginningwithParagraphThree(3),LineTwelve(12)withtheword“In”andendingonPageFour(4),LineSixteen(16)withtheword“Agreement”bedeletedandthefollowinginsertedinitsplace:“InnoeventshallactualservicesperformedunderthisAgreementbeinexcessofThreeHundredFiveThousandandNo/100Dollars($305,000)fortheperiodofJuly1,2011throughJune30,2015.InnoeventshallservicesperformedunderthisAgreementbeinexcessofThreeHundredForty-FiveThousandandNo/100Dollars($345,000)fortheperiodofJuly1,2015throughJune30,2016.”-1-COUNTYOFFRESNOFresno.CA
